Terms and Conditions Sustain Speed Nutrition

1. Consultation Fees: All consultation fees are to be paid in full within 3 days of receiving the invoice.

2. Payment Methods: Accepted form of payment is via bank transfer, please use your surname as a reference.

3. Invoicing: Invoices will be provided detailing the services rendered, fees charged, and payment due. Invoices will be issued in electronically.

4. Payment Deadline: Payment is due within 3 days of the date of the consultation or as otherwise specified in the invoice.

5. Late Payments: A 5% late payment fee may be applied to invoices not settled within 7 days.

6. Appointment Scheduling and Cancellation: Appointments are scheduled in advance, and cancellations or rescheduling must be made with at least 24 hours' notice. Cancellations made within 24 hours of the scheduled appointment time may be subject to a cancellation fee, as outlined in our Cancellation Policy.

7. Missed Appointments (No-Show): Failure to attend a scheduled appointment without prior notice will result in the full consultation fee being charged.

8. Insurance and Reimbursement: It is the client's responsibility to check with their insurance provider regarding coverage for dietetic services. The client is responsible for submitting claims for reimbursement if applicable.

9. Confidentiality: All client information and records will be kept confidential, in accordance with relevant privacy laws. Information may only be disclosed with the client's explicit consent or as required by law. For more information please see the Sustain Speed Nutrition Privacy Policy.

10. Changes to Appointments: The dietitian reserves the right to reschedule or cancel appointments due to unforeseen circumstances. In such cases, the client will be promptly notified, and reasonable efforts will be made to accommodate the client's schedule.

11. Termination of Services: The dietitian reserves the right to terminate services for any reason, with reasonable notice provided to the client.

12. Client Responsibility: Clients are responsible for providing accurate and complete information during consultations, updating the dietitian on relevant changes in their health status, and following any recommendations or guidelines provided.

13. Modification of Terms: These terms and conditions may be modified or updated at the discretion of Sustain Speed Nutrition. Clients will be notified of any changes.
